COVID Self-Test Kits Available
COVID testing at the Lab is self-service. The testing kits are located in the Building 50 auditorium and Potter Street lobbies. An appointment to drop off a kit is suggested, and the kits will be picked up Monday through Thursday at 9 a.m. for testing by the vendor. https://sites.google.com/lbl.gov/covid-portal/covid-testing
Declaring Vaccination Status
The Lab requires all employees and affiliates working onsite to complete the COVID-19 Vaccine Declaration Questionnaire through the Medical Health Portal. If you were vaccinated at the Lab or already uploaded your COVID vaccination card, you do not need to complete the questionnaire. http://clinic.lbl.gov/
